Treatment Plan

TREATMENT PLAN - Susan Martin (ID# 563971821)

Mrs. Martin will receive the following treatment:

- Activator Adjustments.

Activator adjustments will be applied to directly influence joints which have been identified as functioning aberrantly. Due to the severity of the muscle spasm and tenderness at this time, activator adjustments will be utilized to initiate correction of identified subluxations. The specific structures being adjusted will be noted on the record of the daily visit.

MODALITIES AND INSTRUCTION

- Out of office cryotherapy.
Mrs. Martin will be given instructions on home icing which is necessary to reduce edema related to her condition and prepare for and compliment the chiropractic adjustment.

- Electrical myo-stimulation (EMS).

- TENs therapy.
TENs therapy will be provided to the patient to reduce spasm, provide palliative relief and break the aberrant neural cycle currently associated with the patient's injury.

LIMITATIONS

At this time I am imposing the following limitations on Mrs. Martin to avoid aggravation of her current condition:

- Work must be limited to 8 hours or less a day.
- No lifting of any object over 10 pounds.
- Until further notice no sports activities, no jarring activities, no activities which may result in a fall, and no activities which may result in a blow to the body or head.
- No repetitive motions involving the neck and arms.
- Until further notice no vacuuming, no mopping, and no reaching over their head.

FREQUENCY OF TREATMENT

Susan will be seen approximately 2 to 3 times a week. This frequency of care will continue for 4 to 5 weeks. At that time she will receive an examination to reevaluate her condition and evaluate her progress.

PROGNOSIS

Rendering a prognosis at this time would be premature due to the lack of information available on Mrs. Martin's future response to treatment.
